How to access and make the most of various services of the Practice

Register with the Practice

New patients welcome!

Patients who are resident in Merton are welcome to register with the practice. Please ask at reception, call us on 0208 648 0822, or contact us to find out more.

Each new patient is given the opportunity to undergo a medical check-up and is asked to complete a new patient questionnaire which is totally confidential.

Make an Appointment

The surgery uses an appointment system and to make an appointment either call in at reception or telephone during surgery opening hours, 020 8648 0822 or 020 8648 0354. You can also make an appointment using the Online Appointment Booking facility.

Request a Home Visit

All doctors in the practice make home visits. Arrangements for visiting are made each day according to the availability of doctors and pressure of work in the surgery. Home visits late in the day and at night are made by the doctor who is on duty.

Obtain a Repeat Prescription

If you need regular treatment your clinician may authorise the issue of repeat prescriptions. When you receive your prescription you will also receive a tear-off portion giving details of your repeat medication/s. For ordering repeat prescriptions please either tick, on the tear off portion, the medicines you wish to order or complete a prescription request form which you can find on ground floor reception desk and hand to receptionist. Please allow 48 hours (two working days, not including weekends or Bank Holidays), when requesting a repeat prescription.

Obtain a Sickness Certificate

You do not require a doctor’s sickness certificate for any illness lasting six days or less. Your employer may, however, require you to complete a self-certification form SC1 which is available from your employer.

Obtain Telephone Advice

Sometimes you may want brief advice on a health problem or medication and simply wish to talk to the doctor on the telephone. Please ask the receptionist for the times when this is convenient. It is very helpful to let the receptionist know what it concerns so that we have the necessary records available.

Obtain Test Results

We regularly receive the results of blood tests, x-rays and correspondence from local hospitals. They are checked by your doctor who will contact you if there are any important findings. There is no need to contact the surgery for results of investigations unless instructed by your doctor.

We constantly strive to give you the best possible care and attention. If you have any suggestions or are unhappy about any aspect of our services please ask to see the practice manager who will be happy to listen to your suggestions/criticisms.
