How to Register with the Practice
Each new patient is given the opportunity to undergo a medical check-up and is asked to complete a new patient questionnaire which is totally confidential.
The check-up is routine and it is important that you take up this offer for your benefit, particularly if you do not attend the doctors very often. You will be given a specimen pot to bring a urine sample to the appointment. This check gives us some important information about your health. The check should only take about 10 or 15 minutes. This initial check-up is carried out by the nurses and helps them to assess your general health and familiarise themselves with your medical history.
Change of Address Details
Please let us know as soon as possible any change of name or address (or telephone numbers) so that we may update our records. This makes it easy for us to get in touch with you in case of an emergency. Also unless you do this, the Health Services Authority may remove you from our list of patients without letting you know.
