Register for Online Services

To register for our online services you will need to complete the form below and provide one form of photo identification. Once your registration has been processed, we will issue your username and password.

Acceptable forms of ID include:

  • Passport

  • Driving licence

  • Birth certificate plus proof of address (e.g., a utility bill or bank statement dated withing the last 3 months)

Once you are registered, you will be able to use the service to:

  • Order your repeat prescriptions

  • See your medical records

  • Review your medications and known allergies

Proxy Access (Family or Carer Access)

If you would like to give consent to your carer or a member of your family to access your medical records, please click here or visit the practice reception to collect a Proxy Access form.  If you choose to collect the form in person, both the patient and the person who is going to have access to the medical records must attend together and bring a valid form of ID.
