New Patient Registration Form

If you would like to register with the Practice, please complete our online registration form below. This form is only for patients who are not currently registered with the Practice.

Alternatively, you can visit the Practice to complete our paper-based registration forms.

To register as a new patient, you must live within our practice boundary. We only accept patients whose council tax is paid to the London Borough of Merton.

Please note that we are a Safe Surgery. This means that lack of ID, proof of address, immigration status, or language barriers will not prevent you from registering.

If you do not know your NHS number, please obtain it from your previous GP practice. We require this to ensure your registration is processed correctly and to allow us to transfer your medical records. Your registration cannot be completed without your NHS number.
